To register at the practice, you must:
- Complete a registration form
- Provide 2 forms of identification (1 photographic if possible)
- If you are unable to provide a form of identification please discuss this with our practice manager.
- You must live within our practice area if you do not you should register with a more local GP practice.
At present we are only able to facilitate the registration of immediate family members residing at the same address as a registered patient, new births and those assigned to us by the health board. We do accept temporary registrations (those who expect to stay less than 3 months in the area) if individuals are in need of care.
If you are finding it difficult to registrar at a GP practice in Falkirk you may contact NHS practitioner services on 01312757957.
Once you have been accepted as a patient, your medical records will be transferred to the practice and you will be sent a new medical card. Your medical records may take up to several weeks to arrive at the practice.
If English is not your first language, the practice has access to interpreters via the telephone language line service. Please inform reception staff along with details of your primary language and this assistance can be provided. If you are planning to discuss more than one problem and require the use of the interpreter service please help us by asking for a double appointment.
